Care Plan Coordinator
Hello, my name is Mindy Pearse. I am the RN Care Plan Coordinator at Montgomery Nursing and Rehabilitation Center. I have worked here since March 2002.
As the Care Plan Coordinator, I assess each resident upon admission and every quarter after that. At the time of assessment I determine the resident's needs and write an individualized care plan accordingly. All the information is compiled on computer software and sent to the state database for evaluation.
Also at the time of assessment, the resident and family is invited to attend a care plan conference with the interdisciplinary team including the nursing, dietary, social services, and activity departments. This is to ensure that communication lines are open between resident, family, and facility. Individualized needs are addressed at this time and changes are made if necessary.
Even though this is a scheduled meeting every quarter, please be assured that our doors are always open for questions or concerns at any time, not just during the care plan meetings.
